Extractions – Replace the Missing Tooth

When removing a tooth it is important to consider what will be done with the empty space after that tooth is removed. Wisdom teeth are in the back of the mouth and aren’t part of your bite, so that site will heal with no complications. But, for example, if a first molar is removed, there could be serious complications for your bite if it isn’t replaced. Your teeth tend to move in to close any spaces and they tend to erupt until they meet some resistance. That is why, when you have a full dentition, they all touch each other and bite together at the same time. If a tooth is missing, that screws up the whole system.

The diagram on the right shows what happens if a missing first molar isn’t replaced. The tooth in front of it will tend to tip backward a little, creating a gap between it and the tooth in front of that. The tooth behind will tip forward, usually quite a bit, trying to close that space. The tooth above will super-erupt. This causes three problems. One is that the teeth that are tipping will be weakened. They are now subject to stresses they aren’t designed to withstand, which may lead to eventually losing those teeth also. Second, they create food traps that can be difficult to clean, leading to gum disease. But the most serious problem is that they will disrupt your otherwise harmonious bite, which can lead to muscle spasms and TMJ disorder.

Tooth Replacement Options

For replacing a single missing tooth, either a dental bridge or a dental implant is an excellent option. If there are multiple missing teeth, they can be replaced with dental implants. If your budget doesn’t allow that, a much more economical option would be a removable partial denture.
